By submitting this form, you authorize ANNA to prepare, use, reproduce, publish, and exhibit the name, art, and story in connection with traditional, digital, and social media advertising and marketing. You acknowledge that the artwork or photograph(s) submitted is property of ANNA and may be used by ANNA as it sees fit. You will receive no compensation and waive any approval of the finished product. Artwork will remain the property of ANNA and will not be returned to the artist.
This artwork submission release agreement (“the Agreement”) is made by the individual identified by name, email, and member number above (“the Grantor”) and the American Nephrology Nurses Association (“ANNA”), with mailing address of 200 East Holly Ave., Sewell, New Jersey 08071.
WHEREFORE, the Grantor is choosing to create and submit artwork for the ANNA Art Contest, as part of the ANNA 55th Anniversary National Symposium, to occur in April 2023; and
WHEREFORE, the Grantor intends to fully transfer any and all ownership and rights to the artwork it submits as part of the ANNA Art Contest; and
THEREFORE, the Grantor, in exchange for good and valuable consideration, expressly agrees as follows:
- Transfer of Ownership. Grantor expressly agrees that by submitting the artwork, Grantor is transferring all rights and interests of any kind, including copyright of the Artwork, to ANNA upon submission for the Art Contest.
- Grantor expressly understands, accepts, and consents to ANNA’s full ownership and legal rights to the Artwork upon submission by the Grantor, including any future copyright, trademark, or income generated by ANNA. ANNA shall have all legal authority to disseminate or use the Artwork in its sole discretion.
- Submission shall be defined as the Grantor emailing, faxing, mailing, or hand delivering the Artwork to ANNA or any agent of ANNA.
By signing below, I confirm that I have read and agree to the above Terms of Transfer in their entirety and I certify that the work submitted is my own, that no other individual or parties owns the work I submitted, that no one has obtained a copyright of the submitted work, and that the permission being granted does not violate any prior agreements I have made with anyone else. I have signed this Agreement after having an opportunity to review same with legal counsel.
